include:<\/p>\n<ul>\n<li>identification of the operator, his residence or registered office address,<\/li>\n<li>identification of the main operator or identification of the responsibilities of the individual operators of designated parts of the installation for operating the installation in accordance with environmental legislation, in the case referred to in Article 183b,<\/li>\n<li>the address of the plant on whose premises the installation is operated,<\/li>\n<li>information on the title to the installation,<\/li>\n<li>information on the type of installation, the equipment and technology used, and the technical characteristics of the sources and emission sources,<\/li>\n<li>assessment of the technical condition of the installation,<\/li>\n<li>information on the type of activity conducted,<\/li>\n<li>description of the assumed operational alternatives of the installation,<\/li>\n<li>a general process diagram with a mass balance and the types of materials, raw materials and fuels used, relevant to environmental requirements,<\/li>\n<li>information on the energy used or produced by the installation,<\/li>\n<li>the magnitude and sources of emissions or sites of emissions - actual and proposed - during normal operation of the installation and under abnormal conditions, in particular such as start-up and shut-down operations,<\/li>\n<li>the conditions or parameters characterising the operation of the installation, determining when start-up is completed and when shut-down of the installation commences,<\/li>\n<li>the proposed procedures for monitoring the technological processes relevant to the environmental protection requirements and, in particular, the measurement or recording of emissions and fire protection requirements in the case of a waste permit which includes the collection or treatment of waste,<\/li>\n<li>the declared date and method of cessation of operation of the installation or of the designated part thereof which does not pose a risk to the environment, where cessation of operation is anticipated within the period for which the permit is to be granted,<\/li>\n<li>the declared total duration of continued operation of the installation, where this has an impact on the determination of the environmental protection requirements, and the declared means of documenting this duration of operation,<\/li>\n<li>the declared commissioning date of the installation (if applicable),<\/li>\n<li>the length of time for which the permit is to be issued,<\/li>\n<li>the tax identification number (NIP) and REGON number of the waste holder,<\/li>\n<li>specification of the types of waste to be generated, taking into account their basic chemical composition and properties,<\/li>\n<li>determination of the quantities of each type of waste expected to be generated per year,<\/li>\n<li>identifying ways of preventing or reducing waste and its negative environmental impact,<\/li>\n<li>a description of further waste management, including collection, transport, recovery and disposal of waste,<\/li>\n<li>indication of where and how and the types of waste stored.<\/li>\n<\/ul>\n<p>\n<\/div><\/div><\/div><\/div><div restriction of liberty or a fine<\/strong>. The same penalty shall apply to an operator who uses the installation in violation of the conditions of the permit issued.<\/p>\n<\/div><\/div><\/div><\/div><div class=\"panel panel-default wpb_accordion_section group  no-block-padding\"><div class=\"panel-heading wpb_accordion_header ui-accordion-header\"><p class=\"panel-title font-weight- text-   icon-size-rg\"><a data-toggle=\"collapse\" data-parent=\"#accordion_784027753\" href=\"#1714063845004-7-0174670291783319174677697129217467933878481746798637638\"><span>When will I receive a waste generation decision?<\/span><\/a><\/p><\/div><div id=\"1714063845004-7-0174670291783319174677697129217467933878481746798637638\" class=\"panel-collapse collapse\"><div class=\"panel-body wpb_accordion_content ui-accordion-content half-internal-gutter single-block-padding\">\n\t\t\t\t\t\t<div class=\"uncode_text_column\" ><p>If you submit the correct application, the authority will give you a decision <strong>within one month of receipt<\/strong>. The procedure is therefore identical to the original permit.<\/p>\n<p>Remember that until the new decision comes into force, you must operate under the terms of the previously issued decision!
